HOME HEALTH CARE NURSE

 

EMPLOYMENT STATUS:      Exempt, salaried.

 

SUMMARY OF DUTIES: Provides comprehensive nursing service to individuals and families in the home and community health setting, accepting primary care responsibilities and coordinating with other disciplines and community agencies.   Provides health services to special community groups.

 

JOB REQUIREMENTS: Baccalaureate degree from an accredited College of Nursing, (one year clinical experience preferred); or graduate of an accredited school of Nursing.  Home care or community health nursing experience preferred.  Licensure as a registered nurse in Iowa.  Car, driver's license and adequate insurance as outlined in the personnel policies.  A physical examination is required, completed at the time of employment with a statement from the physician of the ability of the employee to perform the duties of the job.  Annual TB testing is required.  All offers of employment are conditioned upon the result of a criminal background check conducted by the Iowa Division of Criminal Investigation, as required by law.

 

 

HOME CARE AIDE

 

EMPLOYMENT STATUS:  Non exempt, Salaried

 

SUMMARY OF DUTIES: The Home Care Aide (HCA) provides assistance with activities of daily living, treatments which are ordinarily self administered, and instrumental activities of daily living which an individual is unable to carry out due to a temporary or permanent disability.  The HCA also functions as a trained and substitute caregiver when a household experiences stress or crisis precipitated by absence, incapacity or limitations of the usual caregiver.  Home Care Aide services focus on maintaining and improving the individual’s ability to care for herself or himself, and on promoting community living with a personal feeling of wellbeing in a safe environment.    Components of service include but are not limited to: personal care services, essential shopping, housekeeping and laundry, respite care, money management, consumer education, parenting skills and family management.

 

JOB REQUIREMENTS: Successful completion of at least 60 hours of Home Care Aide training and fifteen hours of practicum, or coursework equivalent in content and depth, and/or successful completion of a competency test as approved by the National HomeCaring Council, the Iowa State Department of Health and by this agency.  Required is a physical examination completed at the time of employment and as requested by VNA with a physician’s statement of the employee’s ability to perform the duties of the job.  Annual TB testing is required.  Car, adequate liability insurance and current driver’s license are required.  All offers of employment are conditioned upon the result of a criminal background check conducted by the Iowa Division of Criminal Investigation, as required by law.  

 

PHYSICAL THERAPIST

 

EMPLOYMENT STATUS:  Full Time, Exempt, salaried

 

SUMMARY OF DUTIES:  Provides comprehensive physical therapy services to individuals in the home setting for the purpose of developing and restoring function through rehabilitative physical therapy.  Accepts primary care responsibilities and coordinates with other disciplines and community agencies.

 

JOB REQUIREMENTS:  Graduate from an approved School of Physical Therapy, licensed to practice in the State of Iowa.  One year experience providing direct patient services in a health care setting.  Car, driver's license and adequate insurance as outlined in the personnel policies.   Required is a physical examination completed at the time of employment.  Annual TB testing is required.  All offers of employment are conditioned upon the result of a criminal background check conducted by the Iowa Division of Criminal Investigation, as required by law.
